In limestone country it is routine. The rock carries water through caves and solution channels rather than filtering it evenly through soil, so entry and discharge points can be far apart with nothing at the surface connecting them. Grading tells you very little here.

Often a solution feature that has settled at the surface. It collects runoff rather than shedding it and connects to the network below, so filling it without knowing what it drains into usually just moves the problem somewhere less convenient.
