What it is
Water damage arriving from above: a roof failure, a supply or waste line in a ceiling cavity, or condensation from equipment in an attic. The defining feature is that the stain marks where water left the assembly, not where it entered, so treating the stain treats the symptom.
When it is needed
A stain that appears or keeps growing, a ceiling that sags or feels soft, a light fitting that drips, or a mark that only shows after particular weather. Wind-driven rain entering at one point and appearing several feet away is ordinary rather than unusual.
What it typically involves
The work runs backwards from the stain. Water travels along joists, ducts and pipe runs until something interrupts it, so the entry point is found by following the path uphill rather than by opening the ceiling where the mark is. Weather conditions matter to the diagnosis: a leak that only appears in wind-driven rain points at a wall or flashing detail rather than the roof field. Once found, the cavity and the insulation above the ceiling are dried - insulation that has been wet loses its performance and usually comes out - and repair waits until readings show the assembly is dry. Painting over a stain before that point conceals the evidence while the moisture stays where it was.
